Revert r5027 as it does not work anymore (surflock is mandatory now) and prevents the card from getting emms (see #2130).

idletimeout now a valid option for cccam-reader (but keepalive keeps
them connected)
idle handling improved, now only ecms/cs changes idle value

1.10 : reader_viaccess , backport r5962 which cause some improvement on some viaccess cards like: NTV Plus , TNTSAT , CSAT/C+ Mips tuxbox as user reported.
